What You'll Do
The Certified Nursing Assistant (CNA) provides direct care and support services to participants in both the PACE Center and home settings. The CNA assists participants with activities of daily living, mobility, meal service, restorative activities, personal care, and household management while promoting participant dignity, safety, independence, and quality of life. This position works collaboratively with the interdisciplinary team to ensure participants receive comprehensive, person-centered care in accordance with their individualized care plans.

Essential Duties And Responsibilities
Participant Care
- Assist participants with activities of daily living including feeding, bathing, dressing, grooming, toileting, incontinent care, and personal hygiene.
- Assist participants with transfers, ambulation, positioning, and mobility using assistive devices and mechanical lifts as needed.
- Assist with oral feeding and meal support.
- Observe and report changes in participants' physical, emotional, cognitive, or behavioral condition to the appropriate supervisor.
- Provide companionship, emotional support, and meaningful engagement to participants.
- Assist with therapeutic, recreational, and social activities.
- Serve meals, snacks, and nutritional supplements in accordance with dietary guidelines.
- Assist with participant transportation and community outings as assigned.
- Accompany particioants to medical appoints as assigned.
- Maintain a clean, safe, and organized environment throughout the center.
- Support emergency preparedness activities and respond appropriately during emergencies.
- Provide personal care services in participants' homes according to the care plan.
- Perform light housekeeping duties including laundry, changing bed linens, dusting, sweeping, and cleaning kitchen and bathroom areas.
- Plan and prepare simple meals and assist with meal cleanup.
- Accompany participants to medical appointments and community activities as assigned.
- Travel to participant homes and provide services to multiple participants as scheduled.
- Maintain accurate and timely documentation of participant care and services provided.
- Communicate effectively with participants, caregivers, family members, and members of the interdisciplinary team.
- Participate in team meetings, training sessions, and care planning activities.
- Assist in maintaining equipment, supplies, and work areas.
- Follow all PACE, CCANO, CMS, OSHA, HIPAA, and safety regulations.
- Participate in required training, in-service education, and staff meetings.

What You'll Bring:
- High School Diploma or GED.
- Current Louisiana Certified Nursing Assistant (CNA) certification.
- Current CPR certification.
- Minimum one year of experience working with older adults or in a healthcare setting.
- Strong interpersonal, communication, and customer service skills.
- Ability to safely transfer and assist participants with mobility needs.
- Ability to work independently and as part of an interdisciplinary team.
- Experience in adult day health care, home health, long-term care, or a PACE program.
- Valid driver's license and reliable transportation.
- Serve Safe Food Handler Certification or willingness to obtain upon hire.
- Ability to lift, transfer, reposition, and assist participants safely.
- Ability to stand, walk, bend, stoop, reach, and perform physical tasks throughout the workday.
- Ability to travel between participant homes and PACE Center locations as needed.
