What are the responsibilities and job description for the Registered Nurse position at Catawba Valley Medical Services (CVMS)?
Reports to: Director of Nursing
Qualifications/Educational Requirements
- Resides in North Carolina
- Current CPR certification
- Comfortable Using Microsoft Suite
- Excellent oral and written communication skills
- Valid Driver’s License with reliable transportation
- Knowledge of home care principles, practices, and regulations in North Carolina
- Graduate of an accredited nursing program (Associate’s or Bachelor’s degree in Nursing)
- Ability to work with and have sensitivity to the needs of the geriatric population (aged 60 )
- Must hold a current, unencumbered Registered Nurse (RN) license in the state of North Carolina
- Two (2) years’ experience with the AxisCare software system preferred or similar Home Health software
- Minimum of one year of clinical nursing experience, preferably in home health, hospice, or community health settings
Client Assessment and Care
- Perform comprehensive nursing assessments to identify the physical, psychosocial, and environmental needs of clients.
- Develop and implement individualized care plans based on the assessment findings, client needs, and physician’s orders.
Monitoring and Evaluation
- Monitor client progress and response to care, making necessary adjustments to the care plan and interventions.
- Document client care and progress accurately and timely manner, maintaining compliance with company policies and state regulations.
Client and Family Education
- Educate clients and their families about disease processes, treatments, medications, and self-care techniques to promote independence and safety in the home.
- Provide guidance and support to caregivers to enhance client care.
Coordination of Care
- Collaborate with the interdisciplinary team, including physicians, therapists, social workers, and other healthcare professionals, to coordinate care and ensure a holistic approach to client health.
- Communicate effectively with other members of the care team to ensure continuity and quality of care.
Compliance and Documentation
- Maintain compliance with all applicable North Carolina state statutes and regulations governing home care services.
- Ensure all documentation is accurate, complete, and submitted in a timely manner according to agency and regulatory standards.
- Report significant change in client status to the Director of Nursing promptly, consistent with the client’s needs.
Emergency and On-Call Duties
- Respond promptly to client emergencies and provide appropriate interventions.
- Assumes responsibility and accountability for the practice of professional nursing in accordance with the State Nurse Practice Act and standards for home health nursing.
