The successful candidate will serve as the key partner in each patient's plan of care, working strategically in patients' homes while promoting dignity and independence while collaborating with physicians, caregivers, and the broader clinical team. This position is ideal for professionals passionate about making a tangible difference in the lives of patients by supporting their recovery and empowering them to live safely and independently at home.
Responsibilities:
- Identify and meet patients’ goals and needs through comprehensive assessment and intervention.
- Develop and implement individualized care plans using a variety of treatment techniques.
- Deliver quality, evidence-based, and personalized care to patients in their homes.
- Motivate and support patients during therapy to help them achieve optimal function.
- Promote healthy lifestyles by improving patients’ strength, flexibility, balance, and coordination.
Required Skills:
- Licensed/Registered as a Physical Therapist in the state of Florida.
- 1 year minimum of skilled Physical Therapy experience.
- Bachelor's Degree from an accredited Physical Therapy program.
Nice to Haves:
- Experience working in home health care, acute care, or a clinical setting.
- Familiarity with electronic health records or home health documentation systems.
- Interest in patient education and community health promotion.
