Position Overview
Catalyst for Change is seeking a compassionate, dependable, and motivated Care Coordinator to join our growing team.
The Care Coordinator will work directly with individuals to identify their needs, develop person-centered goals, connect them with community resources and services, and provide ongoing support as they work toward greater stability and independence.
We are looking for someone who is organized, personable, self-motivated, and passionate about helping others navigate challenges and achieve their goals.
Responsibilities
- Build positive and professional relationships with clients.
- Identify individual needs, strengths, barriers, and goals.
- Develop and maintain individualized, person-centered care plans.
- Assist clients in accessing community resources and supportive services.
- Coordinate services with healthcare providers, behavioral health providers, social service agencies, housing resources, employment services, and other community organizations.
- Advocate for clients and help them develop self-advocacy skills.
- Monitor progress toward established goals and update care plans as needed.
- Maintain regular communication with clients and members of their support team.
- Complete case notes, care plans, assessments, and other required documentation accurately and within required timelines.
- Maintain client confidentiality and appropriate professional boundaries.
- Manage an assigned caseload while maintaining organization and timely follow-through.
- Participate in team meetings, trainings, and other required activities.
- Follow Catalyst for Change policies and all applicable program and state requirements.
Qualifications
- Strong communication and interpersonal skills.
- Compassionate, nonjudgmental, and person-centered approach.
- Ability to work independently and manage multiple clients and responsibilities.
- Strong organizational and time-management skills.
- Comfortable working with individuals who may be experiencing behavioral health, substance use, housing, financial, or other life challenges.
- Ability to maintain confidentiality and professional boundaries.
- Basic computer and documentation skills.
- Valid driver’s license and reliable transportation may be required for community-based services.
- Must meet applicable background check and program eligibility requirements.
Schedule

This is a community-based position. Some flexibility in scheduling may be required based on client needs.
Compensation is based on experience, qualifications, and applicable program requirements.
