This individual will conduct surveys of facilities, including state licensure and complaint surveys. The HFSN will be responsible for preparing written survey documentation. Under general supervision, surveys facilities to determine compliance with state licensure requirements and Department directives, program policies and procedures. Travels to conduct routine health and safety surveys to determine compliance with state and federal requirements. Interviews facility residents, staff, administrators, health care providers and other individuals. Observes patient care provided at facilities. Reviews medical records, policies, standard operating procedures and other facility documents related to licensure and/or regulatory compliance. Requires licensure as a Registered Nurse in the State of Illinois.Requires appropriate, valid driver’s license.Requires ability to travel in the performance of duties with overnight stays as appropriate.Overtime is a condition of employment, and you may be requested or required/mandated to work overtime including scheduled, unscheduled, or last-minute overtime. This requires the ability to work evenings and weekends.Must reside in JoDavies, Stephenson, Winnebago, Boone, Carroll, Ogle, Dekalb, Whiteside, Lee, Rock Island, Mercer, Henry, Bureau, Putnam, or LaSalle County.
