PRN. Part-Time, or Full-Time | Monday–Friday | 8:00 AM – 4:00 PM

Your Role
Complete in-home physical therapy evaluations
Create and deliver customized therapy plans
Provide one-on-one sessions tailored to each patient’s goals
Educate patients and families on exercises for continued improvement
Collaborate with nurses, physicians, and other care providers
Document patient progress accurately
What You Bring
Valid Illinois Physical Therapist license
Strong assessment and clinical skills
Compassion, empathy, and great communication abilities
Reliable transportation to patient homes
Home health experience preferred—but we’ll provide support if you’re new to it
