Location: Peoria, Princeton, and surrounding communities and counties.
Position Summary
Castor Home Nursing is seeking a compassionate and motivated Registered Nurse (RN) Care Manager to join our growing clinical team.
This field-based position works with patients, families, nurses, and clinical leadership to support care coordination, clinical oversight, staff development, and quality patient care throughout Central Illinois.
A company vehicle is provided for work-related travel.
Responsibilities
- Provide clinical oversight and care coordination for assigned patients.
- Complete assessments, admissions, supervisory visits, and other clinical visits as needed.
- Support and maintain strong relationships with patients, families, and field nurses.
- Assist with nurse orientation, education, training, and competencies.
- Monitor clinical documentation and plans of care.
- Collaborate with clinical leadership and other members of the care team.
- Support quality, compliance, and continuity of care.
- Perform other clinical and administrative responsibilities as assigned.
Qualifications
- Active Illinois RN license required.
- Pediatric, Private Duty Nursing, Home Nursing, Home Health, or care management experience preferred, but not required.
- Strong communication, organization, and relationship-building skills.
- Ability to work independently and manage multiple priorities.
- Comfortable with a field-based position and regional travel.
- Valid driver's license and acceptable driving record required.
