We are seeking a dedicated and skilled Licensed Practical Nurse (LPN) who is passionate about making a real difference in the lives of residents every day. In this role, you will provide hands-on nursing care, administer medications and treatments, and play a key role in monitoring resident health and ensuring timely, high-quality care.
As a valued member of our care team, you will collaborate closely with nurses, CNAs, and providers to deliver safe, compassionate, and person-centered care. Key Responsibilities
Deliver direct nursing services aligned with individualized care plans, physician directives, and established clinical protocols
Conduct ongoing evaluation of resident status, documenting findings and promptly reporting any changes to the appropriate members of the care team
Prepare and administer prescribed medications and therapeutic interventions in accordance with regulatory standards and scope of practice
Coordinate with interdisciplinary team members to support continuity of care and achieve optimal clinical outcomes
Provide education and guidance to residents and their families regarding treatment plans, medications, and overall health management
Oversee and direct the work of CNAs and support staff, ensuring care is delivered safely, effectively, and in accordance with established standards
Complete thorough and accurate documentation within the electronic medical record system in a timely manner
Contribute to facility-wide initiatives related to infection prevention, quality improvement, and resident safety
Identify and respond appropriately to urgent or emergent situations, following established clinical procedures
Maintain compliance with professional standards, confidentiality requirements, and all applicable federal and state regulations
Graduate of an accredited practical nursing program with a current, active LPN license in good standing with the State of Wyoming
Current Basic Life Support (BLS) or CPR certification
Demonstrated ability to deliver compassionate, resident-focused care across a variety of clinical conditions and acuity levels
Proven ability to work collaboratively within a multidisciplinary healthcare environment
Strong critical thinking and decision-making skills with the ability to adapt to changing clinical situations
Effective communication and relationship-building skills with residents, families, and healthcare professionals
