Overview
We are seeking a dedicated and compassionate Case Manager to join our team, providing critical support and guidance to individuals facing behavioral health challenges, addiction issues, and social service needs. This role involves coordinating care, developing individualized plans, and connecting clients with necessary resources to promote stability and growth. The ideal candidate will have experience in crisis intervention, social work, and behavioral health, with a strong commitment to empowering clients through mentorship and supportive services.
Responsibilities
- Conduct comprehensive assessments to identify client needs related to mental health, addiction, child welfare, and housing programs such as Section 8 and Medicare.
- Develop personalized case management plans that align with clients’ goals and available community resources.
- Provide crisis intervention and crisis management support to ensure client safety during emergencies.
- Offer addiction counseling and behavioral health support to facilitate recovery and well-being.
- Utilize motivational interviewing techniques to encourage positive behavior change and foster client engagement.
- Coordinate with social service agencies, healthcare providers, and community organizations to facilitate access to services such as childcare, workforce development, and housing assistance.
- Supervise or mentor junior staff or interns as needed, ensuring quality service delivery.
- Maintain accurate documentation of client interactions, progress notes, and service plans in compliance with organizational policies.
- Advocate for clients’ needs within various systems including child welfare and healthcare programs like Medicare.
Qualifications
- Proven experience in crisis intervention, crisis management, or behavioral health settings.
- Background in social work, addiction counseling, or related fields preferred.
- Strong knowledge of community resources such as Section 8 housing, child welfare services, Medicare, and workforce development programs.
- Excellent communication skills with the ability to motivate and mentor clients effectively.
- Experience supervising staff or mentoring interns is a plus.
- Ability to handle sensitive situations with empathy and professionalism while maintaining confidentiality.
- Certification or training in motivational interviewing is desirable.
