Key Responsibilities:
- Assess patient needs and develop personalized care plans to help achieve therapeutic goals.
- Provide one-on-one and group therapy sessions to patients based on their treatment plans.
- Collaborate with an interdisciplinary team of healthcare providers to ensure optimal care delivery.
- Maintain accurate and up-to-date patient records and documentation.
- Educate patients and families about therapy processes and ways to support recovery.
- Monitor patient progress and adjust treatment plans as needed.
Qualifications:
- Active and valid licensure or certification for the relevant position (PT, PTA, OT, COTA, ST).
- Proven experience in a clinical or healthcare setting is preferred.
- Strong interpersonal and communication skills to interact effectively with patients, families, and team members.
- Ability to work both independently and as part of a team.
