The LOFT is more than just a place to live—it’s a structured, supportive home where youth work hard to complete school, build job skills, and prepare for independent living. During their stay of up to 24 months, residents save a portion of their income to help them secure stable housing after graduation. The Case Manager is a vital part of this transformative journey, providing youth with the guidance, support, and encouragement they need to build a successful, self-sufficient future.
About the Role:
As a LOFT Case Manager, you will be the primary counselor for youth enrolled in the program. You’ll help ensure their daily needs are met, guide them in achieving personal and educational goals, and support their transition toward independence. Your work will include case management, individual counseling, life skills development, and coordination of essential services such as healthcare, employment, and education.
This full-time position (40 hours/week) requires flexibility with variable shifts (days, swing, weekends, and holidays) and includes on-call responsibilities.
Key Responsibilities:
- Conduct assessments, intakes, and develop individualized service plans.
- Facilitate independent living skills groups and positive recreational activities.
- Provide counseling support to help youth meet treatment goals and prepare for self-sufficiency.
- Coordinate and transport youth to medical, school, employment, and housing-related appointments.
- Maintain thorough case files and behavioral documentation.
- Build strong relationships with residents, families, caseworkers, and community partners.
- Support youth in finding employment, completing education, and saving for future housing needs.
- Assist with daily living supervision and behavior management with compassion and consistency.
- Contribute to a positive, structured home environment that fosters growth and resilience.
