Cascade AIDS Project in SW WA is excited to announce that we are hiring skilled and team oriented Medical Case Manager. The individual works on Cascade AIDS Project’s SW Washington team to provide individuals living with HIV and those at high risk of HIV through the continuum of HIV-related and other relevant systems of care. This role focuses on helping clients to create and maintain engagement in medical care and medication adherence to strive to achieve viral load suppression. This requires maintaining an awareness of cultural challenges and developing effective relationships with a diverse client population and community partners throughout the service and surrounding areas

This is a Full-Time at 32hrs a week role working at our Vancouver location in SW Washington. The schedule for this role is Monday-Thursday 3-days in Office, 1 day work from home 9:00am-5:00pm.
Starting pay is $34.51 an hour which is equivalent to just over $57,428 a year. The Medical Case Manager’s Pay increases with tenure and the top of the pay scale is $39.33. This is a union represented position, so the compensation, hours, benefits and conditions of work are collectively bargained.

Who You Are
  • Bachelor’s Degree in the social services field (Social Work, Psychology, Public or Community Health) with two (2) years’ professional experience providing direct case management services OR minimum of three (3) years’ professional experience providing direct case management services
  • Prior experience providing case management services with knowledge of care coordination, client advocacy, and community service navigation.
  • Proficiency with tech and tools and software, including Microsoft Office (Word, Excel, PowerPoint, Outlook) and willingness to learn new software
  • Excellent written and oral communication skills
  • Experience working with ethnic, racial, economic, and sexually diverse populations and persons who have experienced homelessness, persons with a mental illness and/or substance addiction
  • Strong understanding of issues relevant to gay/bi/trans men, people living with HIV and AIDS, communities of color, and the LBGTQ community and how those issues affect or influence service delivery

What You’ll Do
  • screening clients for psychosocial needs, developing goal plans, implementing effective interventions and monitoring client progress
  • assisting clients with applications for insurance, public assistance benefits, etc., and monitoring the process from initiation through formal decision
  • providing information and referral to a wide range of community services
  • providing advocacy within multiple settings including educating clients to self-advocate when appropriate; providing problem solving interventions as needed to assist client’s completion of their goals
  • working closely with providers and inter-disciplinary team to facilitate client care and resolve short-term needs.
