Schedule: Floating Between Dansville Residential and Hornell IP (varied shifts including days and evenings)
Location: 45 Maple Street Dansville, NY 14437 and 7309 Seneca Rd North Hornell, NY 14843.

The Licensed Practical Nurse (LPN) provides direct nursing care and clinical support within residential and inpatient substance use disorder (SUD) programs as part of an integrated behavioral health model. This role supports continuity of care, clinical quality, and client engagement while collaborating with a multidisciplinary team to address co-occurring substance use, mental health, and medical needs. All services are delivered in compliance with NYS OASAS, OMH, SAMHSA, public health regulations, and agency policies.

Essential Responsibilities

  • Provide compassionate, respectful, and trauma-informed care that supports client safety, dignity, privacy, and confidentiality.
  • Collect and document vital signs, health histories, and medication information.
  • Administer and monitor medications per provider orders, including mental health and SUD medications, long-acting injectables, and MAT/OTP-coordinated medications as appropriate.
  • Assist with medication ordering, inventory control, delivery, and secure storage in accordance with regulatory standards.
  • Monitor clients for medication side effects and withdrawal symptoms; promptly report concerns to the RN or provider.
  • Support coordination of care for clients receiving integrated SUD, mental health, and OTP services.
  • Manage specimen collection, documentation, and lab coordination (e.g., drug screens, TB testing, COVID testing).
  • Provide health education and reinforce provider instructions to clients and families.
  • Observe, report, and document changes in client condition to the treatment team.
  • Participate in shift reports, care team huddles, and interdisciplinary meetings.
  • Maintain accurate, timely documentation in the Electronic Health Record (EHR).
  • Adhere to infection control, safety, confidentiality, and public health standards.

Additional Responsibilities

  • Participate in case reviews, supervision, quality improvement activities, and required trainings.
  • Assist with onboarding and training of new nursing or direct care staff as assigned.
  • Support client safety, medication adherence, and treatment engagement.
  • Maintain Qualified Health Professional (QHP) status as required.
  • Perform other duties as needed to support program operations.

Cross-Training & Flexibility

  • Participate in cross-training across residential and inpatient SUD programs.
  • Maintain nursing competencies across assigned bedded programs.
  • Provide flexible coverage based on staffing needs while ensuring safe, effective care.
  • Contribute to a collaborative, supportive team environment.

Knowledge, Skills & Abilities

  • Foundational knowledge of substance use disorders, withdrawal management, and co-occurring mental health conditions.
  • Familiarity with medication-assisted treatment (methadone, buprenorphine, naltrexone).
  • Strong documentation, observation, and communication skills.
  • Proficiency with EHR systems and basic computer applications.
  • Ability to work effectively in a fast-paced, 24-hour treatment environment.

Physical Requirements

  • Ability to use a computer and perform clinical documentation.
  • Frequent lifting up to 10 pounds; occasional lifting up to 25 pounds.
  • Ability to manage stressful situations and respond appropriately to client crises.
  • Maintain professionalism and situational awareness to ensure safety.

Work Environment

This position operates within a 24-hour residential and inpatient treatment setting. Employees must comply with infection control standards and maintain a tobacco-, alcohol-, and drug-free workplace.

Qualifications

  • Graduate of an accredited Licensed Practical Nurse (LPN) program.
  • Active, unrestricted New York State LPN license.
  • One year of nursing experience in behavioral health, substance use, or medical settings preferred.
  • Valid driver’s license and reliable transportation.
  • Successful completion of required background and clearance checks.
