What are the responsibilities and job description for the Case Manager - Comprehensive Community Support Worker, CCSS position at Casa De Mi Alma?
Casa De Mi Alma is looking for a compassionate Comprehensive Community Support Services (CCSS) Worker to provide direct, one-on-one support services to children and youth with emotional, behavioral, or developmental needs. CCSS will also have the opportunity to support adults at the practice. This position is split between school-based services and office-based/community services at our practice. You’ll play a key role in helping clients build the skills needed to function effectively at home, school, and in the community.
Key Responsibilities:
- Deliver CCSS interventions focused on building daily living skills, coping strategies, social skills, and community integration.
- Collaborate with therapists, school staff, families, and other providers to support the client’s treatment goals.
- Provide school-based support, including behavior modeling, and individualized interventions.
- Provide support at the private practice, including community-based work and client sessions.
- Maintain accurate and timely documentation of services provided in compliance with Medicaid and agency guidelines.
- Participate in team meetings, treatment planning sessions, and ongoing training.
Qualifications:
- High School Diploma or GED required; Associate’s or Bachelor’s degree in social work, psychology, education, or related field preferred.
- Must meet requirements for CCSS worker under New Mexico Medicaid requirements and be a Certified CCSS worker/or be willing to get certified
- Experience working with children, adolescents, and families in educational or therapeutic settings preferred.
- Strong communication and interpersonal skills.
- Ability to work independently, manage time effectively, and adapt to different environments.
- Must pass background checks and have reliable transportation.
Preferred Skills:
- Bilingual (English/Spanish) a plus
- Knowledge of trauma-informed care
- Familiarity with school systems and and in individual care
- Proficient in technology and computer skills

Pay: $20.00 - $25.00 per hour
Expected hours: 15 – 30 per week
