What are the responsibilities and job description for the Case Management Associate - Community Support .5 FTE position at Carver County?
Under the supervision of the unit supervisor, the Case Management Associate performs supportive and adjunctive case management and community support services. This position will provide CSP groups, one-on-one support, transportation, and similar group activities. Hours of the position are 8:00 a.m. - 4:30 p.m. selected days, Monday through Friday. This may also include potential early evening hours once per week. Starting pay range is $23.44 - $29.30 per hour depending on qualifications. This part-time position is eligible for single medical insurance, PERA (Public Employees Retirement Association) pension plan, and prorated holiday pay and PTO. TYPICAL DUTIES AND RESPONSIBILITIES: Ability to do and document CSP activity groups. Perform outreach to potential clients including assistance in completing forms, providing assistance in understanding the nature of available services and how potential clients may qualify for and benefit from these services. Assist case manager in work with clients on a variety of housing supports, including tenancy rights, working through conflicts with neighbors and/or landlords, and assist clients in keeping current with rent payments. At direction of case managers or case management supervisor, lead a variety of supportive and rehabilitative groups to adults with serious and persistent mental illness. Other duties as assigned.
