The Job Description

General Statement of Duties
Plans and manages the Postpartum/Newborn Visiting Program, including assisting in establishing program goals, policies, procedures, and guidelines.

Distinguishing Features of the Class
An employee in this class provides intermediate level professional nursing work in providing primary, preventive, and rehabilitative care to individuals and families in Public Health programs. Employees independently demonstrate and implement techniques of nursing care and counsel and promotion of health and prevention of disease through outreach. Employees will be expected to assist in data collection and review in any project that they are associated with. Employees may manage a program-level support activity; participate in program or agency-wide planning and quality assurance activities; contribute observations or data related to patient or community trends or practices, ideas for improved program performance; participate in record audits and other program reviews. Work is performed under the direct supervision of the Public Health Nursing Supervisor I and is evaluated by review of records and reports, in conferences, approval of seminars and observation.

Duties and Responsibilities
Essential Duties and Tasks
  • Plans and manages the Postpartum/Newborn Home Visiting Program, including assisting in establishing program goals, policies, procedures, and guidelines. Acts as coordinator of program. Work requires specialized knowledge in the area of Maternal and Child Health.
  • Independently performs nursing assessments outside the Health Department requiring strong assessment skills.
  • Identification and referral-acts as a liaison with hospital, physicians, and other programs to locate qualified clients for this program. The goal is to visit all Medicaid eligible postpartum women, no later than 60 days after delivery.
  • Spends the majority of the time providing advanced and independent nursing service to clients in the home environment who have multi-disciplinary needs and who reside in a socially critical environment.
  • Collaborates with PCMs and CC4Cs serving potential clients.
  • After receiving referral for service, makes appropriate contact with families to facilitate Postpartum/Newborn Home visit.
  • Critical assessment of needs made in family’s home and appropriate referral as needed. Follow-up with family to assure needs were met.
  • Cooperates with nursing supervisor to set programmatic or operational goals. Participates in the development and implantation of local polices in adherence to state mandates.
  • Program policies and procedures are carried out according to local policy and as mandated by the State Division of Maternal and Child Health. Maintains a ledger of services rendered. Assists with billing procedures as needed.
  • Assist with orientation of new personnel as needed.
  • Required to perform duties with maximum accuracy to assure that appropriate services are received by clients. Uses expertise with guidelines set forth by the Maternal and Child Health Division as reference for operational directives.
  • Maintains accurate client records that are available for follow-up and review.

Additional Job Duties
  • Any other duty as necessary, including assisting with any public health emergency including but not limited to natural disasters, man-made disasters, influenza pandemics (e.g. H1N1) or other outbreaks of communicable diseases, disaster surveillance and response, to ensure the efficiency of the Health Department.
  • Assist in the staffing of Red Cross Shelters.

Knowledge, Skills and Abilities
  • Considerable knowledge of, and skill in, the application of nursing theory, practice, principles, and techniques employed in the field of public health and related programs.
  • Thorough knowledge of clinical procedures and basic physiological processes.
  • Thorough knowledge of County and department policies and procedures.
  • Thorough knowledge of applicable Federal and State laws, rules, and regulations.
  • Thorough knowledge of available resources and organizations and the ability to coordinate these as needed.
  • Thorough knowledge of OSHA and CDC policies and procedures.
  • Through knowledge in the use of standard office equipment and associated software (e.g. CureMD, LabCorp Beacon, NCIR, Meditech, NC Tracks, etc.).
  • Thorough knowledge in the use of equipment required for the position.
  • General knowledge of current social and economic problems relating to public health, including health disparities.
  • Ability to plan, coordinate, and oversee the work of others.
  • Ability to deal tactfully with others and to exercise good judgment in appraising situations and making decisions.
  • Ability to work in partnership with patients and with other service providers to elicit needed information and to maintain effective working relationships.
  • Ability to record accurately services rendered and to interpret and explain records, reports, and medical instructions.
  • Ability to solve problems within scope of responsibility.
  • Ability to exercise sound judgment in analyzing situations and making decisions.
  • Ability to communicate effectively in oral and written form.
  • Ability to make arithmetic computations such as, rates, ratios, and percentages.
  • Ability to deliver correct dosing of vaccines and medications, as needed.

Physical Requirements
  • Work requires the regular exertion of up to 10 pounds of force and occasional exertion of up to 50 pounds of force.
  • Work regularly requires sitting and using hands to finger, handle or feel, frequently requires walking, speaking or hearing, reaching with hands and arms and repetitive motions and occasionally requires standing, climbing or balancing and stooping, kneeling, crouching or crawling.
  • Work requires close vision, distance vision, depth perception, color perception, and peripheral vision.
  • Vocal communication is required for expressing or exchanging ideas by means of the spoken word and conveying detailed or important instructions to others accurately, loudly or quickly.
  • Hearing is required to perceive information at normal spoken word levels and to receive detailed information through oral communications and/or to make fine distinctions in sound.
  • Work requires preparing and analyzing written or computer data, operating machines and observing general surroundings and activities.
  • Work occasionally requires exposure to fumes or airborne particles and frequently requires exposure to blood borne pathogens and may be required to wear specialized personal protective equipment.
  • Work is generally performed in a moderately noisy location (e.g. business office, light traffic).

Desirable Education and Experience
Graduation from a two-year college with a degree in nursing, or related field and two years of experience in nursing including one year in public health, or an equivalent combination of education and experience.

Special Requirements
A current license to practice as a Registered Nurse in North Carolina by the NC Board of Nursing.
