Summary
The Transitional Care Registered Nurse (TCRN) functions on the multidisciplinary team as an advocate, interpreter, educator for Carson Tahoe Health patients and community. Serves as a liaison throughout the facility and in the community regarding services provided by Carson Tahoe Health. The Transitional Care Nurse will collaborate with Primary Care Providers, Inpatient Case Management and Urgent Care Providers. As well as collaborate with interdisciplinary teams, and resources to assess, coordinate, implement, and evaluate services required to promote quality outcomes and efficient resource management.
Qualifications
- Required
o Commitment to a Bachelor’s Degree in Nursing program within two years of hire
o American Heart Association (AHA) Basic Life Support (BLS)/Cardiopulmonary Resuscitation (CPR) for healthcare providers Certification
o NVCI/CPI Training (Non-Violent Crisis Intervention) initial certification within six (6) months of hire, refresher completed annually
o Minimum 5 years’ of acute care experience
o Minimum of 2 years’ ambulatory care setting experience
