Description

This is a full-time, FLSA non-exempt position with Carson City Health & Human services located in Carson City, NV. Under general supervision, provides necessary support and guidance to clients to ensure that they attain and maintain opportunities. Position is contingent upon grant funding continuing to be renewed/approved.

This is a grant-funded position and, as such, employees are responsible for knowing abiding by the rules of the grant. This position will continue to be funded as long as the grant is renewed.

Employees May Choose Between The Following

Employer-Paid PERS: $22.7003 - $31.7805 hourly / $47,216.52 - $66,103.44 annually

Employee/Employer-Paid PERS: $27.3854 - $38.3396 hourly / $56,961.58 - $79,746.42 annually

  • Supports and works cooperatively with multi-disciplinary teams working with Human Services clients.
  • Services clients by referring to appropriate services; providing assistance in enrolling in the services, coordinating services, and providing case management services in reference to continuity of care and after care.
  • Supports individualized goal setting using motivational interviewing and problem-solving strategies; and assists with the creation and implementation of self-management plans to meet client’s goals.
  • Provides appropriate follow-up with clients via phone calls, home visits and outreach visits.
  • Ensures clients receive the services they need and have the appropriate health and social services, education, and information; provides appointment reminders; provides translation assistance; assists with finding appropriate transportation; and may attend medical or social service appointments with clients.
  • Advocates for the client by working with medical and social service providers to help ensure that clients have comprehensive and coordinated care and services.
  • Documents and maintains accurate records of work performed as required by project or program.
  • Maintains confidentiality of files and information in accordance with regulations.
  • Demonstrates ability to use standard office equipment, including a computer, in the course of the work.
  • Demonstrates courteous and cooperative behavior when interacting with public and staff; acts in a manner that promotes a harmonious and effective workplace environment.
  • Seeks professional development.

Education And Experience

High school diploma or GED OR an equivalent combination of education, training and experience as determined by Human Resources.

Required Certificates, Licenses, And Registrations

  • Valid driver’s license
  • Community Health Worker Certificate or must obtain within 12 months of the first day of employment.

Supervision Received And Exercised

Under General Supervision - Incumbents at this level are given assignments and objectives that are governed by specifically outlined work methods and a sequence of steps, which are explained in general terms. The responsibility for achieving the work objectives, however, rests with a superior. Immediate supervision is not consistent, but checks are integrated into work processes and/or reviews are frequent enough to ensure compliance with instructions.

Mobility to work in a typical office setting, use standard office equipment and stamina to sit for extended periods of time; strength to lift and carry up to 20 pounds; vision to read printed materials; and hearing and speech to communicate in person or over the telephone; ability to operate a motor vehicle and safely travel to a variety of offsite locations.

Conditions Of Employment

  • This is a grant funded position. Position is contingent upon grant funding continuing to be renewed and approved.
  • Continued employment is contingent upon all required licenses and certificates being maintained in active status without suspension or revocation.
  • Any City employee may be required to stay at or return to work during emergencies to perform duties specific to this classification or to perform other duties as requested in an assigned response position. This may require working a non-traditional work schedule or working outside normal assigned duties during the incident and/or emergency.
  • Employees may be required to complete Incident Command System training as a condition of continuing employment.
  • New employees are required to submit to a fingerprint-based background investigation which cost the new employee $47.00 and a drug screen which costs $36.50. Employment is contingent upon passing the background and the drug screen.
  • Carson City participates in E-Verify and will provide the Social Security Administration (SSA) and, if necessary, the Department of Homeland Security (DHS), with information from each applicant’s Form I-9 to confirm work authorization. All candidates who are offered employment with Carson City must complete Section 1 of the Form I-9 along with the required proof of their right to work in the United States and proof of their identity prior to starting employment. Although the medical and recreational use of marijuana is lawful in Nevada, state law allows the City to prohibit the use of marijuana in the workplace and does not require the City to modify the job or working conditions of a person who engages in the medical use of marijuana if the job or working conditions are based upon the reasonable business purposes of the City. An employee who engages in the recreational use of marijuana and through a laboratory test is positive for the use of marijuana will be terminated.
