Position Overview

The Social Services Director is responsible for overseeing and coordinating the psychosocial needs of residents in accordance with federal, state, and facility guidelines. This role ensures residents are supported emotionally, socially, and in planning for successful transitions of care. The ideal candidate will be compassionate, highly organized, and experienced in long-term care or healthcare social work.

This position plays a vital role in ensuring resident rights, dignity, and quality of life are maintained throughout their stay at Oakview Nursing and Rehabilitation.

Key Responsibilities

  • Complete comprehensive psychosocial assessments for new admissions and ongoing resident evaluations
  • Develop, implement, and update individualized social service care plans
  • Participate in interdisciplinary care plan meetings and resident conferences
  • Coordinate admissions, discharges, and safe transition planning to home or other levels of care
  • Assist residents and families with adjustment to placement, coping strategies, and emotional support
  • Serve as a liaison between residents, families, physicians, and external agencies
  • Ensure compliance with all federal and state regulations related to long-term care social services
  • Maintain accurate, timely, and thorough documentation in resident records
  • Assist with advance directives, guardianship arrangements, and resident rights education
  • Support grievance resolution and advocate for resident needs and preferences
  • Coordinate community resources, financial assistance programs, and discharge planning services
  • Participate in surveys, audits, and quality improvement initiatives as needed
  • Maintain confidentiality in accordance with HIPAA regulations

Qualifications

  • Previous experience in Social Services, Human Services, Healthcare, or Long-Term Care strongly preferred
  • Knowledge of Medicare/Medicaid regulations and long-term care requirements preferred
  • Strong communication, documentation, and organizational skills
  • Ability to work effectively with residents, families, and interdisciplinary teams
  • High level of empathy, professionalism, and problem-solving ability
  • Ability to manage multiple priorities in a fast-paced healthcare environment
  • Experience with discharge planning and care coordination strongly preferred

Schedule & Setting

  • Full-time position
  • Primarily weekday schedule (may require flexibility based on resident needs, admissions, or discharge planning requirements)
  • Skilled nursing and rehabilitation facility environment
