MDS Coordinator- Requirements:
High school diploma or equivalent.
Nursing Experience: 1 year
RN/ LPN License.
Responsibilities of MDS/Care Plan Coordinator:
- Actively participate in the development of a comprehensive, individualized plan of care for each resident.
- Ensure resident plan of care is completed, updated, and reviewed in accordance with the RAI guidelines and changes in resident care/needs.
- Assume responsibility for implementation of Clinical and Prospective Payment System Minimum Data Set completions schedule
- Direct, organize and schedule required resident assessments and related documentation for Medicare, Medicaid, managed care and Health Maintenance Organization (HMO) reimbursement
- Coordinate and oversee all functions of the MDS assessment process to assure compliance with federal and state requirements
- Participate in resident level of care determination upon admission
- Review reports of timely completion of residents' MDS assessments and care plans and take follow up action as necessary
- Analyze Quality indicator reports for potential resident concerns and problems and review residents' care plans to be sure this information is included
- Will maintain a current knowledge base regarding state and federal regulations relating to the RAI process, care planning and the prospective payment system (PPS).
