Title: Housing Stability Coordinator
FLSA Status: Non - Exempt
Job Summary: The Housing Stability Coordinator will provide services to participants in the form of timely response to individuals who are homeless or at risk of homelessness in order to ensure they are referred to the appropriate community agency or program. This individual will also provide information to participants on area resources available to them. This position will provide Outreach, Intervention and Housing Transition to eligible participants. This staff person will track and document services provided to ensure accurate data, this staff person will provide recovery-focused, culturally relevant services to the individuals served by the grant. The person who holds this position must have a philosophy of promoting and fostering housing stability for the participants.
Essential Duties/Expectations:
- Possess knowledge and experience with strategies such as trauma informed care, harm reduction, motivational interviewing, critical time Intervention and Housing First practices.
- Assess housing barriers of individuals and families at risk or experiencing homelessness to determine housing and service needs.
- Make referrals to Homeless Trust, City of Miami Outreach or other shelter facilities (where appropriate).
- Conduct a Strength Based Needs Assessment to determine housing needs
- Provide outreach to engage individuals or work on site with residents to ensure long-term housing sustainability through modeling, motivation, and coaching to help them reach self-sustainability by developing a housing stability plan.
- Provides direct services of Housing Transition services, including: Processing applications and referrals for assistance and housing opportunities, assisting eligible applicants in finding and obtaining a suitable apartment that meets the needs of the participant and the guidelines for a housing subsidy if needed, with emphasis on the participant choice of housing whenever possible.
- Provide/assist in referrals for community resources for affordable housing opportunities and/or completing housing applications
- Provide mediation and advocacy with landlords on the participant’s behalf to develop a workable plan to obtain housing.
- Focus on tenancy supports, such as interacting positively with landlords and neighbors.
- Create and maintain consistent communication channels, both verbal and written, between several parties (i.e. tenant, landlord, referral source, collaborating agencies).
- Apply knowledge of residential lease contracts to educate participants and residents of their rights and responsibilities.
- Maintain accurate data of all participants served including quarterly performance outcome measure.
- Attend scheduled workshops, trainings and meetings as required
- Actively participate in weekly/monthly supervision
- Maintain all records and information in accordance with Carrfour policies and funding sources
- Other duties as assigned to support and ensure the success of the program
- Reports to the Scattered Sites Assistant Program Director and/or Program Director
Minimum Requirements:
Skills:
- Understanding of working directly with at risk and formerly homeless individuals and families and at-risk populations by treating all individuals with respect and are able to build rapport by promoting empathy and compassion with patience and consistency.
- Pays close attention to detail and demonstrates strong organization skills.
- Strong critical thinking skills and ability to problem solve.
- Effective communication skills backed by detailed written documentation and comprehensive listening skills.
- Maintains a professional demeanor and maturity, good judgment, quick learner, and proactive
- Ability to multi-task, prioritize and manage time efficiently.
- Proficient in Excel, Adobe, Microsoft programs, Outlook, and understanding of database applications, including the use of formulas, functions, data import/export and creating charts.
Education/Experience:
- Knowledge and experience with strategies such as trauma informed care, harm reduction, motivational interviewing, critical time Intervention and Housing First practices
- Bachelor’s degree in social work, mental health, psychology, or related field required, and a minimum of three years related field experience preferred.
- Fluency in the English language is required. Spanish or Creole speaking ability is an added asset, but is not required.
- Knowledge or understanding of tenant’s rights and responsibilities as well as “strengths based” case management.
- Concentrated record keeping, budgeting, and mathematical skills; ability to produce required reports to federal, state. and local government agencies and funding sources.
- Demonstrated knowledge of community resources, social service agencies, and landlords.
- Sensitivity to cultural and socioeconomic characteristics of population served.
- A commitment to empowering others to solve their own problems.
- A conviction about the capacity of people to grow and change.
- The ability to establish and set appropriate limits with persons served to help them gain skills and confidence.
- The ability to work collaboratively with other personnel and/or service providers or professionals.
