Role Description The Licensed Clinical Social Worker (LCSW) will provide direct clinical services to individuals,and groups, including psychosocial assessments, treatment planning, and evidence-based interventions.
This full-time, hybrid role is based in Charlotte, NC, with a mix of on-site work and some work-from-home flexibility as appropriate for client needs and organizational policies. Daily responsibilities include conducting clinical evaluations, providing individual and group therapy, coordinating care with medical and behavioral health providers, and facilitating referrals to community resources.
The LCSW will engage in case management, crisis intervention, and discharge planning, while maintaining accurate and timely documentation in the electronic health record. The role also involves participating in interdisciplinary team meetings, contributing to quality improvement initiatives, and supporting client education and advocacy to promote overall wellbeing.
Qualifications
- Strong foundation in Social Work practice, including client-centered, trauma-informed, and culturally responsive approaches.
- Experience or training in Medical Social Work, including working collaboratively within hospital or healthcare settings.
- Proficiency in Case Management, including coordination of services, resource linkage, and advocacy across systems of care.
- Clinical experience in Mental Health, including assessment, diagnosis (where permitted), and evidence-based therapeutic interventions.
- Knowledge of Behavioral Health principles, including treatment planning, crisis intervention, and collaborative care with multidisciplinary teams.
- Active, unrestricted LCSW license (or equivalent) in the state of North Carolina.
- Excellent communication, documentation, and organizational skills, with comfort using electronic health records and telehealth platforms.
- Ability to work independently and as part of an interdisciplinary team in a hybrid environment, demonstrating reliability and professional boundaries.
- Experience in outpatient, or community mental health settings is preferred; bilingual or multilingual abilities are a plus.
