Carolina Outreach is seeking a Forensic Community Support Team (CST) Registered Nurse to provide compassionate, integrated care for justice-involved adults living with co-occurring mental health and substance use disorders.
Build Your Purpose.
As a Forensic CST RN, you will work closely with the Forensic CST Team, providing medication support, health education, and coordination of physical and behavioral healthcare in both office and community settings. You will conduct health assessments, monitor adherence to prescribed medications, and support clients in managing chronic health conditions and the side effects of psychotropic medication. You will also engage with detention centers, courts, and re-entry partners to ensure continuity of care during transitions and supports diversion from incarceration when clinically appropriate.
Position Highlights:
- Conduct nursing assessments and monitor medication effectiveness and physical health needs
- Provide medication education, administration support, and adherence monitoring
- Coordinate care with healthcare providers, prescribers, and pharmacies to support integrated treatment
- Deliver community-based services in homes, jails, courts, and shelters to ensure continuity of care
- Participate in treatment planning, crisis intervention, and team collaboration to promote recovery and reduce hospitalization or incarceration

Compensation: $72,666 to $80,410
- Final compensation is determined by factors such as education, licensure level, experience and other qualifications.

Education:
- Graduation from an accredited nursing program.

Certification / Licensure Requirements:
- Must be a Registered Nurse (RN) currently licensed to practice in the state of North Carolina.

Experience / Knowledge Requirements:
- Experience working with adults with co-occurring mental health and substance use disorders is strongly preferred.
- Prior experience in community-based settings and/or with justice-involved populations is also preferred.
