The Nurse Navigator provides assessment and coordinates supportive services for residents transitioning between levels of care at Carolina Meadows and/or external health facilities or acute care settings. Additionally, this position is responsible for providing supportive follow up as necessary following Community Response events from residents in Independent Living, including evaluating the need for additional supportive services that will allow residents to reside safely within the Carolina Meadows community. The Nurse Navigator serves as a guide to connect residents with the community resources necessary to meet their dynamic needs. The position will also support the Carolina Meadows Medical Practice with additional clinical support on an as needed basis.
Required Education and Experience: Graduate of an accredited school of nursing. Two to four years of long term care with geriatric medical services and transdisciplinary team experience. Current licensure as a Licensed Practical Nurse or Registered Nurse in North Carolina. Current CPR certification.
Preferred Education and Experience: Experience caring for those with dementia or cognitive impairment, Skilled Nursing Care and/or admission process, responding to medical emergencies and experience in assisting individuals with navigating medical and care needs.
