The Pediatric Care Coordinator plays a vital role in our primary care medical home, working closely with families and physicians to ensure timely access to necessary care, comprehensive understanding of care plans, and continuity of services. Our team's goal is to promote the well-being of children and their families through effective communication, community partnerships, and education on various topics.
Key Responsibilities:
- Enhance communication among providers, family, and community partners
- Promote access to community services and self-management skills among families
- Provide education on child development, parenting, nutrition, health/wellness, safety, and other relevant topics
The ideal candidate will demonstrate knowledge of comprehensive, community-based, family-centered, and culturally sensitive care coordination principles. They will facilitate family access to medical home providers, staff, and resources, and assist in identifying patients with special healthcare needs.
Care Coordination Services:
- Assess child/patient and family needs, strengths, and assets
- Screen for child's developmental progress and provide follow-up if delays are identified
- Initiate family contacts and create ongoing processes for families to determine/request care coordination support
We seek a highly skilled professional with excellent communication and interpersonal skills, who can work effectively in a team environment. The successful candidate will possess a Bachelor's Degree in a related field, such as nursing, social work, or child development, and have at least three years of relevant experience in community-based pediatrics or primary care.
