Duties And Responsibilities
Compliance Management:
- Responsible for enrolling and coordinating patient care.
- Ensure the CCM program adheres to all Medicare rules and regulations.
- Stay updated on changes in Medicare policies and implement necessary adjustments to the program.
- Develop and conduct training programs for staff involved in the CCM program.
- Manage and optimize workflows to ensure efficient and effective program operations.
- Provide ongoing support and guidance to staff to maintain high standards of care.
- Monitor key program metrics and patient outcomes to assess the effectiveness of the CCM program.?
- Analyze data to identify areas for improvement and implement strategies to enhance program performance.?
- Prepare and present regular reports on program metrics and outcomes to the Director of Quality and Population Health.
Responsible to:
- Directly supervised by Director of Quality and Population Health
- Care Managers
- Works in cooperation with front-office and clinical support staff, other provider organizations, and third-party payers.

Education
In addition, this position requires:
- Bachelor's degree in healthcare administration, nursing, or a related field.
- Minimum of 3 years of experience in healthcare program management, preferably in chronic care management. Strong knowledge of Medicare rules and regulations. Excellent organizational, communication, and leadership skills. Proficiency in data analysis and reporting.
- RN preferred
- Ability to work collaboratively with healthcare professionals and staff.
- Strong problem-solving and critical-thinking abilities.
- Detail-oriented with a focus on quality and compliance.
- Proficient in using healthcare management software and tools. EPIC experience preferred.
- Have the hand-eye coordination and manual dexterity needed to operate a computer, telephone, copier and medical equipment
- Required to talk and hear
- Vision abilities required for this job include close vision, distance vision, color vision, peripheral vision, depth perception, and ability to adjust focus
- Full-time position with occasional travel to healthcare facilities.
- Office-based with some remote work flexibility.
