Carolina Caring is seeking a Field-Based RN to provide comprehensive palliative and serious illness care across home, facility, and telehealth settings. This role is responsible for assessing patient needs, identifying and closing care gaps, and coordinating services such as durable medical equipment, home health, and specialty support. The RN will triage and escalate patient concerns, support care transitions, and educate patients and families on levels of serious illness care to promote understanding and self-management. Working closely with providers and interdisciplinary teams, this role helps optimize care plans, reduce unnecessary hospital and emergency department utilization, and ensure continuity of care. Strong documentation, care coordination, and collaboration skills are essential, along with support for clinic, home, and facility visits as needed.
