Successful candidates will have the following:
- experience in a SNF is ideal
- excellent communication skills
- ability to work collaboratively with residents, their loved ones, and our clinical teams
- ability to work with people of various socioeconomic backgrounds
- a Human Services (or similar) degree is a plus
Your day to day:
You'll assist with care for our residents by coordinating with their care team ensuring their physical and emotional needs are met. You'll work alongside community partners to ensure a safe transition home, when possible. You'll work under the supervision of our Social Services Director to ensure compliance with current state, federal, and local standards.
