Community Health Worker (CHW) Job Description
Reports To: Executive Director
FLSA Classification: Full‑Time, Non‑Exempt
Pay rate: 18.50/hour
Location: Hybrid - on-site at different Cleveland community-based organizations and ~ 20 hours work from home.
Position Summary
Community Health Workers (CHWs) serve as trusted liaisons between health organizations and the public. They educate community members on health issues, connect clients to healthcare and social services, and help reduce barriers to care for underserved and unhoused populations. CHWs provide culturally appropriate health education, assist with Medicaid/SNAP enrollment, and coordinate care through the Better Health Partnership Pathways Hub Program.
Essential Duties & Responsibilities
- Build connections between vulnerable populations and healthcare/social service systems.
- Facilitate navigation of healthcare and government assistance programs.
- Coordinate care transitions and reduce social isolation among clients.
- Provide culturally appropriate health education (chronic disease prevention, nutrition, etc.).
- Advocate for individuals to receive appropriate services.
- Collect and enter client data into the Care Coordination System (CCS) in a timely manner.
- Participate in monthly Pathways Hub meetings and collaborate with care teams.
- Perform basic administrative tasks and maintain accurate client records.
Qualifications
- Certified Community Health Worker licensed under the Ohio Board of Nursing.
- High school diploma or GED required; associate or bachelor’s degree preferred.
- 1–2 years of related experience in community health, social services, or care coordination.
- Valid Ohio driver’s license and access to an insured vehicle.
Knowledge, Skills & Abilities
- Strong interpersonal and communication skills; ability to work effectively with diverse populations.
- Team oriented with effective time management and organizational skills.
- Flexible, empathetic, and able to problem solve under stress.
- Knowledge of community agencies/resources and multi‑system outreach programs.
- Proficiency in Microsoft Office 365 and related applications.
- Ability to think creatively and analytically.
Physical Requirements
- Regular local travel to community sites, partner organizations, and client appointments.
- Ability to lift and carry up to 25 pounds (e.g., outreach materials, supplies).
- Frequent standing, walking, and occasional bending during outreach and client support activities.
- Ability to work in varied community settings, including shelters and public spaces.
