Duties

• Assist residents with activities of daily living such as bathing, dressing, grooming, and mobility support using equipment like Hoyer lifts and transfer methods.
• Monitor and record vital signs, including blood pressure, pulse, temperature, and respiration rates, ensuring accurate medical documentation within Electronic Health Record (EHR) systems.
• Support medication administration under supervision in compliance with HIPAA regulations and maintain strict patient confidentiality.
• Provide specialized care for residents with dementia or Alzheimer's disease, utilizing memory care techniques to promote comfort and safety.
• Collaborate with nursing staff to identify changes in resident conditions and communicate effectively to ensure timely interventions.
• Maintain a clean and safe environment by following aseptic techniques and safe manual handling practices during patient transfers or heavy lifting tasks.
• Document all patient interactions thoroughly in medical records while adhering to regulatory standards for medical documentation.

Requirements

• Valid certification as a Certified Nursing Assistant (CNA) with active state licensure.
• Experience working in long-term care facilities, nursing homes, or assisted living environments preferred.
• Familiarity with EMR/EHR systems such as Cerner or PCC for accurate medical record management.
• Knowledge of medical terminology, vital signs measurement, and basic life support (CPR).
• Ability to perform heavy lifting safely and utilize patient transfer methods like Hoyer lifts effectively.
• Strong communication skills to engage compassionately with residents, families, and healthcare team members.
• Prior experience in dementia care, geriatrics, or behavioral health is highly desirable.
