Responsibilities
- Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support
- Monitor patients’ health status by observing vital signs and reporting any changes promptly
- Prepare nutritious meals tailored to individual dietary needs and assist with feeding when necessary
- Administer medication following care plans and ensure proper documentation in compliance with HIPAA regulations
- Support clients with memory care needs such as Alzheimer’s or dementia, creating a safe and calming environment
- Help with light housekeeping tasks like cleaning, laundry, and maintaining a tidy living space
- Use specialized equipment such as Hoyer lifts or resident lifts to safely transfer clients with mobility challenges
- Collaborate with healthcare professionals to develop and follow personalized care plans
- Provide emotional support and companionship to promote mental well-being
- Follow all safety protocols including CPR and first aid procedures to ensure client safety at all times
Requirements
- Prior experience in home health, assisted living, nursing homes, or group homes is highly preferred
- Knowledge of patient observation, medication administration, and vital signs monitoring
- Certification in CPR and first aid is required; additional certifications such as HIPAA training are a plus
- Experience working with individuals with developmental disabilities, autism, or memory care needs is advantageous
- Ability to perform heavy lifting safely and operate patient transfer equipment like Hoyer lifts
- Strong communication skills and compassionate attitude towards clients of all ages and backgrounds
- Ability to work independently while following detailed care plans accurately
- Valid driver’s license and reliable transportation are preferred for community-based visits
