Role Description
This is a full-time, on-site role for a Hospice RN Case Manager located in Hingham, MA. The Hospice Nurse will provide compassionate and comprehensive care to patients with life-limiting illnesses. Responsibilities include assessing patient needs, delivering palliative care, administering medications, monitoring patient conditions, and collaborating with interdisciplinary teams. The nurse will also provide emotional support and education to patients and their families within home and/or hospice settings, while maintaining proper documentation of care.
Qualifications
- Proficiency in Nursing, Direct Patient Care, and Home Care practices
- Experience in Palliative Care and Medicine with a focus on end-of-life services
- Strong interpersonal and communication skills to support patients and families with empathy and understanding
- Ability to work autonomously and collaboratively with healthcare teams in diverse environments
- RN license in the state of Massachusetts require
- Previous hospice or home healthcare experience is a plus
- Critical thinking, time management, and problem-solving skills
