What are the responsibilities and job description for the Advanced family nurse practitioner position at CARING HEARTS FAMILY MEDICAL CENTERS LLC?
The Advanced Family Nurse Practitioner (FNP) provides comprehensive primary care services to patients across the lifespan, including health promotion, disease prevention, diagnosis, and management of acute and chronic conditions. The FNP works collaboratively with physicians, nursing staff, and administrative personnel to deliver high-quality, patient-centered care consistent with the mission and standards of Caring Hearts Family Medical Centers.
Key Responsibilities:
- Conduct thorough patient assessments, including history-taking, physical examinations, and review of diagnostic tests.
- Diagnose and manage acute and chronic illnesses in patients of all ages.
- Develop and implement individualized treatment plans.
- Prescribe medications, therapies, and other interventions within scope of practice and state regulations.
- Provide preventive care, health education, and counseling to patients and families.
- Collaborate with physicians, specialists, and other healthcare professionals to optimize patient outcomes.
- Maintain accurate, timely, and confidential medical records in accordance with legal and regulatory requirements.
- Participate in quality improvement initiatives and adhere to evidence-based practice guidelines.
- Supervise and mentor nursing staff, medical assistants, or other support personnel as appropriate.
- Stay current with professional development, continuing education, and licensure requirements.
Qualifications:
- Master’s or Doctoral degree in Nursing with FNP specialization.
- Current, active Florida APRN licenses.
- Certification as a Family Nurse Practitioner (FNP-BC or equivalent).
- Minimum of 2 years clinical experience preferred in primary care or family practice.
- Experience with HEDIS, RMA, eClinicalWorks (eCW), and eMedicalPractice highly preferred.
- Strong clinical assessment and diagnostic skills.
- Excellent communication, organizational, and interpersonal skills.
- Ability to work independently and collaboratively within a healthcare team.
Work Environment & Physical Demands:
- Work is performed in a clinical office setting with standard exposure to patients and healthcare materials.
- Ability to stand, walk, and sit for prolonged periods; occasionally lift up to 25 lbs.
