Caring Heart is a 249 bed Long Term and Rehabilitation Care Center located in the heart of Mt. Airy in Philadelphia. Our Rapid Recovery Program is a specially designed clinical excellence program that is personalized to each resident we serve. The program integrates all levels of care from your hospital stay, rehabilitation at our facility, plans to return home, and communication back to your primary physician. Creating this continuum of care helps each patient return home quicker and helps prevent rehospitalization stays. Our team of professionals help residents reach their goals and get them home sooner.
Responsibilities
Responsible for the assessment, treatment planning, therapeutic interventions, re-assessments and discharge planning of patients referred for Speech-Language Therapy as well as Swallow Evaluation and treatment. The Speech-Language Pathologist will also participate in program development, quality improvement, problem solving and productivity improvement in a flexible interdisciplinary fashion. Skills & Abilities: This position is responsible for the assessment and treatment of patients who persistently demonstrate communication deficits in any modality: speaking, listening, reading and/or writing.
Qualifications
- Graduate of a Speech Language Pathology program is required. Bachelors Degree Required; Masters or Doctorate preferred.
- Licensed in Pennsylvania
- CPR Certification
- 1-2 years Long-Term Care Experience preferred
