and Duties: 1. Collaborate with the Pediatrics team to identify, offer
outreach and deliver care for those patients in need of psychosocial
services. 2. Provide resources and behavioral health / social
interventions as needed to restore and preserve the integrity of the
child?s family. 3. Provide links for adolescents to community resources
in the areas of employment, education, housing, substance abuse
services, etc. 4. Maintain a caseload of clients in need of counseling
and case management. Provide clinical supervision and support to
Pediatric Care Coordinators, Community Health Workers, and other support
staff. Maintain and complete in a timely manner all appropriate
paperwork and documentation in compliance with department policy and
procedures. Develop and facilitate psycho-educational or social support
group related to the pediatric services and /or family system. Develop
and build upon existing collaborative relationship with external
agencies related to child and family social services needs. Work
collaboratively within interdisciplinary team and between all
departments. Provide therapeutic interventions including adult and child
individual, family, couple, consultation, collateral interventions
(e.g., Parent Management Training model). Participate in ongoing
continuing education and training activities as needed. Other duties as
assigned to support the mission of the organization. For example, active
participation in pediatric and adolescent health-related grant programs,
quality improvement projects, and teams. Working Conditions: This
position requires the ability to use a computer workstation. \*Salary
