Duties
- Conduct comprehensive assessments of patients’ physical conditions, including evaluating physiology, anatomy, and neurological status.
- Develop and implement individualized care plans tailored to each patient’s needs, including discharge planning and treatment strategies.
- Deliver evidence-based therapies such as orthopedics, neurology, pediatrics, geriatrics, trigger point therapy, and home & community care.
- Document patient progress accurately in electronic health records (EHR) systems while adhering to clinical documentation standards.
- Collaborate with physicians, nurses, and other healthcare team members to coordinate care and adjust treatment plans as necessary.
- Educate patients and their families on physiotherapy techniques, mobility exercises, and injury prevention strategies.
- Manage outpatient rehabilitation sessions at clinics or inpatient units, ensuring safety and adherence to best practices in patient care.
Experience
- Prior experience practicing in a physical therapy role within outpatient clinics or inpatient hospital settings is highly preferred.
- Demonstrated proficiency in discharge planning, treatment plan development, and clinical documentation standards.
- Hands-on experience working with geriatric patients, pediatric populations, or neurological cases is advantageous.
- Knowledge of medical terminology related to orthopedics, neurology, anatomy, and physiology is essential for accurate assessment and documentation.
- Familiarity with electronic health records (EHR) or electronic medical records (EMR) systems for managing patient data efficiently.
- Experience in home health or outpatient rehabilitation centers enhances your ability to deliver comprehensive patient care across various environments.
- Strong client communication skills are necessary for effective patient education and team collaboration.
