What You’ll Do:
- Provide comprehensive care management and coordinate patient care plans across various settings.
- Serve as the key advocate for your patients, ensuring they receive the best possible care.
- Conduct patient assessments, develop personalized care plans, and adjust treatment as necessary.
- Provides health care instructions to the patient as appropriate per assessment and plan of care.
- Monitor patient progress and outcomes to continuously improve care quality and patient satisfaction.
- Educate patients and families on care plans, treatments, and necessary follow-ups.
- This is position allows for work in the field and requires regular weekly travel to client homes throughout WAKE County and surrounding areas*.
- Perform supervisory visits every 90 days (60 days if CAP) for clients in assigned areas.
- Communicate any changes in client condition with primary nurse.
- Communicate any concerns regarding aides with field supervisor.
- Regularly re-evaluates patient nursing needs.

Requirements
- Active RN license in North Carolina or have a multi-state license.
- Graduation from an accredited and approved nursing program, as indicated by school transcript or diploma.
- Minimum of 2 years of nursing experience (case management experience preferred).
- Demonstrated ability to read, write, and effectively communicate in English.
- Strong clinical skills and a deep understanding of case management principles.
- Excellent communication and patient advocacy skills.
- Prior home care experience strongly preferred, but not required.
- A passion for helping others and a compassionate, patient-centered approach to care.
- Solid computer skills: prior experience with electronic medical records (EMR) preferred.
- Must have own reliable vehicle
- Must be flexible
- Ability to travel to cases as assigned.
