Department: Nursing

Minimum Qualifications:

• Current and valid Licensed Practical Nurse (LPN) license in the state of Missouri.
• High school diploma or equivalent required, completion of an accredited practical nursing program.
• Basic Life Support (BLS) certification.
• Demonstrated ability to perform nursing duties in compliance with healthcare regulations and standards.
• Strong communication and interpersonal skills to effectively interact with patients, families, and healthcare team members.

Responsibilities:

• Deliver high-quality, skilled nursing care within the home setting. 
• Follow individualized care plans and physician orders.
• Document timely and accurately.
• Communicate proactively with the care team and Director of Nursing. 
• Provide education and emotional support to clients and their families.
