POSITION: Non-Exempt
Overview:
Provides personalized care and assistance to individuals in the comfort of their home. The primary focus is on supporting clients with daily activities, improving their quality of life, and maintaining their safety and well being. The role requires empathy, patients, and a compassionate attitude toward clients.
Requirements:KEY RESPONSIBILITIES
Personal Care Assistance
Assist clients with daily personal care tasks such as bathing, dressing, grooming and toileting. Help with mobility including transferring clients in and out of bed, wheelchairs or vehicles.
Provide companionship and emotional support to foster social interaction.
Household Support:
Light Housekeeping tasks including cleaning, laundry and meal preparation.
Organize and manage the client's living environment to ensure clean, safe and comfortable space. Grocery Shopping or running errands as needed.
Daily Activities:
Assist with medication reminders, ensure clients take their prescribed medications on time. Support clients with daily routines and activities like reading, writing, light exercise and hobbies.
Monitor client's physical and emotional well-being and report any changes or concerns to family and supervisor.
Transportation
provide transportation to and from appointments, social engagements, or errands as requested.
Safety Monitoring
Ensure the environment is safe, including preventing falls and accidents.
PURPOSE OF POSITION
The primary purpose is to perform needs-based services for clients in their home, in an efficient manner, in accordance with established procedures and as directed by your supervisor. The needs will be assessed and documented in WellSky and documentation of services provided based on those identified needs will be required of you at each visit to the client’s home.
DELEGATION OF AUTHORITY
As a caregiver/housekeeper/transportation assistant, you are delegated the authority, responsibility, and accountability necessary for carrying out your assigned duties. You will be provided with a smart phone app for your daily documentation of services provided and a supervisor to call at any time during your shift when questions arise.
QUALIFICATIONS
Experience
Previous caregiving experience is preferred, but not required. Training will be provided. Ability to work with clients of varying ages, different needs, disabilities and illnesses.
SPECIFIC REQUIREMENTS
- Able to read, write, speak, and understand the English language.
- Able to follow written and oral instructions.
- Able to operate a smart phone and utilize WellSky app for documentation of services rendered.
- Attend and participate in mandatory in-service training programs as scheduled
- Follow attendance policy.
- Perform all tasks in accordance with company policies and procedures and as instructed by supervisor.
- Other related duties and responsibilities that may become necessary or appropriate.
- Collaborate with SC regarding training for needed care and services during the first visit with client
Skills
- Strong interpersonal and communication skills

- Certifications
- CPR and First Aid preferred or willingness to obtain.

Work Environment
- Work will be primarily in client's homes.
- Shifts may vary, depending on client's needs.
- Travel may be required for errands, appointments, and other tasks.
- Client vehicle is used for non-VA clients.
Compensation
Hourly rate pay calculation and benefits as per company handbook.
SERVICE AGREEMENT FUNCTIONS:
Review care plan in WellSky daily for any client changes. Document the care/services provided after each visit.
Ensure documentation reflects the care plan is followed.
