Candidates-In addition to full time positions, CCSN will also consider full time for our Summer Program. Summer Program starts on May 27 through August 10.

Caring Connections for Special Needs (CCSN) improves the behavioral health and wellness of children through a family support approach. We are founded on the belief that we must focus on our children and families and follow our CORE FOCUS to improve the lives of children and families. What makes our services unique is the fact that we offer our enrolled members tools and techniques to benefit the daily life of both the children and families we serve.

CCSN is adaptable to child/family needs and looks to provide family support services such as parenting education, assistance in finding employment, and time management to meet the essential needs of the family. We strive to build trust and develop positive relationships with our members and families.

Responsibilities of the Mentor Position

  • Responsible for coordinating and providing services with agencies and families to meet the needs of the patient according to the treatment plan.
  • Adherence to Program Policy, Documentation of Services.
  • Manage a caseload of 10-15 members that includes children and adolescents under 17.
  • Revenue productivity is established for each mentor following the completion of their mentor training. Each mentor is required to achieve revenue productivity that supports the sites established revenue goals.
  • Responsible for completing monthly coordination of care, parent contact and daily case management in accordance with the CCSN policy.
  • Thorough understanding and maintaining compliance with company policies and procedures.
  • Complies with HIPAA guidelines.
  • Follows Duty to Report and Therapeutic Boundaries
  • Coordinates services internally and externally and in accordance with the members’ treatment plan
  • Attends Behavioral Health scheduled appointments for children enrolled with CCSN including CFT’s.
  • Assists children to meet their needs in accordance with their treatment plan.
  • Actively communicates with pertaining agency about any member concerns, issues, or achievements.
  • Provides services during groups, respite, transportation, school, and in-home services when applicable.
  • Provides direction and follow-up to families and individuals regarding their concerns and issues.
  • Creates a safe environment for the children and their families.
  • Provides information and referral assistance to member’s family.
  • Actively communicates with site staff about any changes with members.
  • Timely documentation of all services provided in accordance with the company’s policies and procedures.
  • Reports all incidents in “real time “to the Supervisor and completes appropriate forms/documentations as directed by the Supervisor.
  • Attends mandatory weekly Supervision facilitated by the CCSN Clinical Director.
  • Communicates with Site Supervisor about hours worked, anticipated overtime, scheduled time off and absences from work.
  • Adhere to assigned work schedule and communicate with your supervisor about any barriers to reporting to work as scheduled.

Key Competencies for a Mentor

  • Must be able to work under pressure and meet deadlines while maintaining a positive attitude and providing exemplary customer service.
  • Ability to work independently and carry out assignments to completion with the parameters of instructions given, prescribed routines and standard accepted practices.
  • Excellent verbal communication skills, including ability to effectively communicate with all age groups of children and parents, guardians, and family members.
  • Must have prior experience documenting services in an electronic health record system.
  • Must have excellent initiative, good judgement, and the ability to make timely decisions.
  • Recognition of priority issues, quick response and execution of plans and processes
  • Understand and be supportive of the organization’s stated mission, purpose, and core values.

Education and Work Experience to be a Mentor

  • A high school diploma or high school equivalency diploma and have four (4) years of full-time behavioral health work experience or:
  • Associates degree and have two (2) years behavioral health experience or:
  • Bachelor’s degree and you have one (1) year of behavioral health experience
  • CCSN prefers a minimum of one year of work experience with children and families

CCSN will also consider applicants that qualify as a Behavioral Health Paraprofessional if the candidate does not currently meet the requirements of BHT.

Required knowledge, skills, and abilities

  • Must be 21 years of age or older, in accordance with Dept. of Health Services R9-20-204.
  • Computer proficiency is required (MS Office- Word, Excel, and Outlook)
  • Must have a current/unexpired Driver’s license and maintain clean driver’s record for the past 36 months.
  • Have reliable transportation
  • Must have and maintain a current/valid/unrestricted Level 1 Fingerprint Card
  • Must obtain CPR/First Aid certification within 60 days of hire and maintain certification throughout employment.
  • TB test within 7 days of hire.
  • Have or obtain Arizona Food Handlers card.
  • Must be able to meet training and agency compliance requirements for the position.
  • Ability and willingness to receive coaching and feedback.

Physical Requirements of the position

An individual may qualify for this position if they can "perform safely" the essential functions of the position with or without reasonable accommodation.

  • Able to perform the essential job functions safely and successfully.
  • Maintain regular, punctual attendance consistent with the ADA, FMLA and other Federal, State and Local standards.
  • Must be able to lift and carry up to 50 lbs.
  • Must be able to talk, listen and speak clearly in person and telephonically.

Mentor Schedule

Monday thru Friday - 11:00 AM to 7:00 PM

School Breaks including Summer Program - schedules modified to Monday thru Friday 9:00 AM to 5:00 PM

Schedules are subject to change based on operational needs.

Pay: $18.00 - $19.00 per hour
