Role Description
We are seeking a Case Manager for a full-time, on-site role located in Newport Beach, CA. The Case Manager will conduct in-home evaluations and assessments of client needs, create personalized care plans, and coordinate caregiver placements. Responsibilities include supporting client-caregiver relationships, maintaining regular communication with clients and their families, providing guidance on care needs, and ensuring continuity and quality of care. The Case Manager will collaborate with caregivers to ensure they are properly prepared and supported to meet client requirements.
Qualifications
- Strong interpersonal and communication skills, with the ability to work effectively with diverse individuals and families
- Experience or skills in conducting evaluations, identifying client needs, and developing personalized care plans
- Organizational and time-management abilities to coordinate caregiver placements and manage schedules effectively
- Understanding of in-home care services and familiarity with client-caregiver matching processes
- Proficiency with basic computer software and administrative tasks
- Knowledge of healthcare industry practices, especially related to caregiving, is a plus
- Ability to work on-site in Newport Beach, CA, with reliable transportation
- High school diploma or equivalent required; additional certifications in caregiving or case management are an advantage
