We are currently seeking compassionate and motivated: We care for patients of all ages and backgrounds who may require daily visits in their homes in the following locations: Dorchester, W. Roxbury, Hyde Park, Roslindale, Dedham, Norwood, Brighton, Brookline, Quincy, Mattapan, Newton, Allston, Cambridge, Brockton, Randolph, Milton, Lynn, Chelsea, East/South Boston, Medford, Somerville (all territories are not expected to be covered per position)
Home Care LPN Responsibilities
- Assess the patient's condition during every visit and document your observations
- Perform different tasks including vital signs and medication review
- Administer medication as prescribed by the patient’s Physicians
- Perform wound care and assess the progress of healing
- Educate patients and their families on proper home health care strategies and procedures
- Coordinate with Physicians and anyone else involved with the patient’s care plan
- Make recommendations for devices/tools that might improve the patient’s quality of life
- Listen to the patient and respond to concerns or requests
- Able to complete clinical documentation with in 24 hours of Patient contact.
Qualifications
- Must have a criminal background check
- State LPN/LVN license required
- 1 year of experience in in-home care is preferred, but we can train the right candidate
- Excellent communication and time-management skills
- Compassionate and friendly demeanor
- Ability to travel to multiple patients per day
- Per Diem
Pay: $30.00 - $33.00 per hour
