Care Angels Home Care is looking for a Director of Pediatric Program for our Licensed Home Care Agency in Brooklyn. As Director of Pediatric Program, you will play an integral role in the daily functions of the agency. The Director of Pediatric Program is responsible for overseeing all of the clinical day-to-day operations including but not limited to Quality Initiatives, Staffing, In-service, Education, On-boarding and Clinical Service Delivery. Must adhere to Federal and State DOH Guidelines in NYS.Director of Pediatric Program Job Description:- Responsible for assessing patient care needs and initial planning for patients referred to home health care services.- Provides guidance and oversight to clinical and non-clinical office and field staff to ensure regulatory compliance, optimal quality of care and case management and timely, accurate documentation.Responsibilities:1. Coordinates, screens and assesses home health care needs of patients referred to the agency.2. Completes intake process within established time parameters and notifies referring party if patient will require alternative discharge plan.3. Collaborates with physicians, nurses and discharge planners to identify patient care needs and to establish the appropriate level of care for patients referred to the agency4. Ensures the completion of a comprehensive, individualized Plan of treatment in accordance with MD orders, patient care needs, insurance guidelines and agency protocol, including the identification of formal/informal supports in carrying out plan.5. Ensures that the needed DME, medical supplies and ancillary services are ordered and documented.6. Schedules and assigns visits for field staff in collaboration with scheduling team.7. Contacts managed care personnel when appropriate to obtain necessary authorizations.8. Conducts case conferences with clinical personnel to ensure appropriate case management and coordination and timely discharge planning.Job Qualifications:- License to practice as a registered nurse or physical therapist (PT) or occupationaltherapist (OT) in NY state, BSN required, Masters/Doctorate preferred.- 3 years of Pediatric OPWDD management level experience in CHHA.Job Type: Full-timeBenefits:Health insurancePaid time offWork Location: In person
