Role Overview: An Occupational Therapist in Home Health assesses and evaluates patients' functional status, activities of daily living, and occupational therapy needs. They provide services within legal scope, using evidence-based techniques, and collaborate with the healthcare team to develop and implement the plan of care.

Key Responsibilities

• Evaluate patients' functional abilities and body systems, documenting CMS requirements.
• Consult with physicians to develop therapy plans.
• Monitor and report patient responses and condition changes.
• Conduct assessments using diagnostic and prognostic tests.

Qualifications & Skills

• Current state licensure in Occupational Therapy.
• Valid driver's license and vehicle insurance or reliable transportation.
• Preferred: CPR certification, multitasking, independence, strong communication, writing, and organizational skills.
