What are the responsibilities and job description for the Home Health Intake Coordinator/ Medical Records position at Carewell Home Health, Inc.?
We are seeking an Intake Coordinator/ Medical Records to join our team! You will work alongside other medical professionals to provide exceptional care to patients. We are open for newly LVN graduates who are looking for experience in the medical field.
Responsibilities:
- Will be responsible for taking Home Health or Hospice referrals from referral sources.
- Ensures that all intakes forms are complete, clear and within the companies scope of service.
- Verifies insurance, enters patient demographics into the computer system, communicates with other departments regarding status of referrals and notifies patients/families regarding coverage and payment responsibilities.
- Speaks knowledgeably of the scope of services the company can provide. Speaks knowledgeably regarding reimbursement issues and all insurance companies the company has active contracts with.
- Is able to take complete, accurate intakes.
- Completes, maintains and distributes the referral log.
- Ensures that insurance verification is completed and authorizations are obtained prior to giving the referral to the Pharmacist or Nursing supervisor.
- Assists with any necessary sub-contracting of services.
- Communicate patient health status to other medical personnel
- Provide preventive health care suggestions to patients
- Participate in standardized health care programs and seminars
- Manages all daily clinical records functions including establishing and implementing clinical records policies.
- Monitors aged unsigned physician orders and submits second and third requests for signed orders to overdue physicians, calls the offices of seriously overdue physicians.
- Ensure accurate and prompt filing and retrieval of clinical records.
- Assist/ help prepare packets for field Nurses (SOC, REC, ROC, DC)
- Ensure completeness of patient’s chart and inform/ notify office clinician and field staff for any missing or incomplete documents.
- Organize patient’s chart/ medical records and arrange in Alphabetical order (per patient’s last name.
Qualifications:
- Previous experience in patient care or other medical fields
- Knowledge of medical terminology
- Ability to build rapport with patients
- Excellent written and verbal communication skills
- Must be able to function in a fast paced environment and possess good people skills.
- Excellent attendance is mandatory.
- We prefer experienced but we are willing to train
- Strong leadership qualities
