Operations Manager – Adult Family Home (AFH/CBRF)CareTruly Healthcare | Milwaukee, WI


Position Summary

CareTruly Healthcare is seeking an experienced, hands-on Operations Manager to oversee daily operations at our Adult Family Home (AFH)/Community-Based Residential Facility (CBRF). This role leads staff, manages scheduling, coordinates resident services, and supports regulatory compliance to ensure safe, compassionate, person-centered care.

Key Responsibilities

Operations and Leadership

• Oversee daily facility operations and maintain a safe, clean, organized environment.
  
  
• Supervise caregivers, CNAs, CBRF staff, and support personnel.
  
  
• Develop staff schedules, monitor attendance and call-offs, and maintain adequate shift coverage.
  
  
• Create contingency plans for staffing shortages and emergencies.
  
  
• Collaborate with HR on recruitment, onboarding, training, coaching, and performance management.
  
  
• Promote teamwork, accountability, and employee retention.
  
  

Resident Services

• Coordinate admissions, assessments, move-ins, transfers, discharges, and healthcare appointments.
  
  
• Collaborate with the RN and Nursing Department on resident service plans, care concerns, incidents, and quality improvement.
  
  
• Communicate with residents, families, healthcare providers, pharmacies, and community partners.
  
  
• Protect resident rights, dignity, privacy, and independence.
  
  
• Provide direct resident care when necessary to maintain resident safety and continuity of services.
  
  

Compliance and Quality Assurance

• Ensure compliance with CareTruly policies, Wisconsin DHS regulations, licensing requirements, HIPAA, and applicable laws.
  
  
• Review resident records, medication documentation, and incident reports.
  
  
• Monitor infection prevention practices and assist with incident follow-up and corrective actions.
  
  
• Partner with HR and Nursing to maintain employee training, certification, and competency records.
  
  
• Support emergency preparedness, licensing surveys, and quality improvement initiatives.
  
  
• Coordinate facility supplies, housekeeping, maintenance, and other operational needs.
  
  

Required Qualifications

• High school diploma or GED.
  
  
• At least two years of supervisory or management experience in an AFH, CBRF, assisted living, long-term care, or healthcare setting.
  
  
• Current Wisconsin CNA certification in good standing on the Wisconsin Nurse Aide Registry.
  
  
• Completion of applicable Wisconsin DHS-required CBRF caregiver training.
  
  
• Ability to perform CNA and CBRF caregiver duties when direct care coverage is needed.
  
  
• Strong leadership, communication, organizational, and problem-solving skills.
  
  
• Ability to maintain confidentiality and communicate effectively in English.
  
  
• Basic computer skills, including Microsoft Office, electronic health records, and scheduling systems.
  
  

Preferred Qualifications

• Associate or bachelor's degree in healthcare administration, nursing, business management, social services, or a related field.
  
  
• CPR and First Aid certification, or ability to obtain within the required timeframe.
  
  
• Valid Wisconsin driver's license and reliable transportation.
  
  
• Bilingual abilities and knowledge of Wisconsin AFH/CBRF regulations.
  
  

Reporting Structure

Reports directly to the Executive Director/Owner and works closely with the RN, Nursing Department, HR, caregivers, residents, families, and healthcare providers. Clinical decisions remain under licensed nursing professionals.
