Registered Nurse
We are seeking Registered Nurses to join our Team! You will be responsible for the assessment, management, and treatment of assigned patients.
Responsibilities Include:
- · Visit clients in the privacy of their residence in designated geographic territories.
- · Schedule and preform monthly supervisory visits
- · Schedule and preform Quarterly Evaluations
- · Complete timely and accurate clinical notes including addressing patient's progress.
- · Evaluate the client status and the skills of workers while the staff member is present.
- · Keeps documentation of all communication with clients, family members and support staff. .
- · Provides patient, family, and/or caregiver education and information pertinent to diagnosis and safety issues.
- · Counsels and educates the patient and family in meeting healthcare and related needs.
- · Write 485/ Plan of Care for patients when reauthorizations are due for patients.
- · Other projects/duties as assigned.
Requirements:
- · Home Care experience preferred
- · Excellent verbal and written communication skills.
- · Highly motivated, organizational, leadership and relationship building skills
- · Ability to work in a very detailed, fast paced environment
- · Currently holds a RN license in PA
- · Valid state driver’s license and car insurance
