I. JOB SUMMARY/RESPONSIBILITIES:
- Develops and implements a plan of treatment to meet patients’ needs in accordance with the principles and practices of a registered nurse. Knowledgeable in community health nursing and rehabilitation concepts. Other key responsibilities include patient/family education, utilization management, and coordination of care and discharge planning.
- Serves as a case manager and supervises home health aides.
- Performs duties in accordance with policies and procedures as established by the agency.
II. TYPICAL PHYSICAL DEMANDS:
- Sitting, finger dexterity, seeing, hearing, speaking.
- Continuous: reaching above, at and below shoulders; repetitive arm/hand motion.
- Frequent: standing, walking, stooping/bending, gripping of objects, and twisting body.
- Occasional: lifting and carrying usual weight up to 50 pounds, pushing/pulling 50 pounds of force.
III. TYPICAL WORKING CONDITIONS:
- Subjected to adverse environmental conditions like continuous exposure to blood borne pathogens.
IV. MINIMUM QUALIFICATIONS:
A. EDUCATION/CERTIFICATION AND LICENSURE:
- Current Hawaii State license as a Registered Nurse.
- Associate's or bachelor's degree in nursing
- Valid U.S driver’s license, vehicle and vehicle insurance as required by Hawaii State Law.
- Driver's abstract reflecting evidence of good driving record.
- Current BLS certification.
B. EXPERIENCE:
- Knowledge of nursing theory and practice to treat patients.
- Knowledge and use of equipment and instruments to administer patient care.
- Skill in identifying problems and recommending solutions.
- Skill in establishing and maintaining effective working relationships with clients, co-workers, and the public.
