Overview
We are seeking a proactive and compassionate Care Navigator to join our dynamic team. In this vital role, you will serve as a bridge between patients and healthcare providers, ensuring seamless access to mental health and medical services. Your expertise will help guide individuals through complex healthcare systems, coordinate care plans, behavioral health needs, and social determinants impacting their health outcomes.
Duties
- Assist clients with intake processes and barriers to accessing services
- Provide consumers with support by connecting with community resources, housing, transportation, and social services or referring to case management for continued on going needs.
- Maintain accurate medical records and manage electronic health records (EHR) systems efficiently
- Conduct client communication with sensitivity, ensuring understanding of healthcare options while maintaining confidentiality in compliance with HIPAA regulations
- Collaborate with healthcare teams to facilitate patient service coordination, including behavioral health and primary care.
Qualifications
- High School diploma/GED required
- Excellent client communication skills; ability serve diverse populations is a plus
- Proficiency in EHR management and data entry to ensure precise recordkeeping
- Ability to work effectively with underserved communities and navigate complex healthcare systems
- Understanding of the concept and importance of whole-person care

Pay: From $16.83 per hour
