Inpatient Case Manager (Onsite/Field)

Northridge, CA

Position Details

  • Pay: $37.50-40/hr (LVN) $45.-50/hr (RN)
  • Schedule: Monday to Friday, 8:30 am - 5:00 pm
  • Location: Hybrid, in-office in Northridge and field visits in the San Fernando Valley

Key Responsibilities

  • Patient Management: Assess, plan, monitor, and evaluate patient care during acute care episodes
  • Collaboration: Work closely with physicians and healthcare practitioners to develop and implement care plans
  • Communication: Keep patients, families, and healthcare teams informed about treatment plans and discharge procedures
  • Documentation: Ensure accurate and timely recording of patient care plans and follow-ups
  • Discharge Planning: Facilitate safe and efficient transitions from hospital to home or other care facilities

Qualifications

  • Current California LVN or RN license
  • At least 2 years of recent experience as an Inpatient Case Manager
  • Proficient in MS Office, with strong computer skills
